Call for Artists!
Design a Logo for a new publication:
Guiding People through Systems (GPS)
Deadline- Friday October 30, 2013

The Ohio Department of Health, in collaboration the Nationwide Children’s Hospital/Autism Treatment Network, Family Voices, University of Cincinnati/UCEDD and The Ohio Medical Home Project is working on a virtual care notebook website, called “Guiding People through Systems” or GPS.
The purpose and goal of the GPS website is to provide an organizational tool for families and individuals to maintain and manage information and records in a notebook fashion.  The GPS will help organize concise information for members of the individual’s healthcare team and will also help families/individuals access resources related to transition, recreation and other services available at the local, state and federal level. This tool has been designed as a stand-alone website that will empower the individual/family/professional partnership.
As part of this initiative, we are offering artists with disabilities in the state of Ohio the opportunity to design a logo for the GPS website.  The logo will be displayed on the GPS website and on any print copies that are generated from the GPS system.
Entries should be submitted as full-color, high-resolution images that are print ready. The logo will be 3” x 3”, so it should be sized accordingly (as a perfect square) and easy to view at a size of 3 inches. Artists are allowed to submit one (1) entryeach.
The winner of this logo design contest will receive recognition in the publication for their design.
Artists should submit their entries and one-paragraph biography to Amy Hess via email (amy.hess@nationwidechildrens.org) by October 30, 2013.
